Google counted them as leads. 28% never became patients.
A dental group with three locations in Texas spent $14K a month on Google Ads. The dashboard showed a $48 cost per lead, but the practice manager felt the calls were getting worse: more price-shoppers, more no-shows.
What the owner was seeing
The group spent $14,000 a month on Google Ads across its three locations, and Google reported a cost per lead of $48. On paper, call volume looked strong.
But the practice manager felt lead quality had dropped: more people calling only to ask about prices, and more booked patients who never showed up.
What was actually going wrong
Google counted any phone call longer than 60 seconds as a lead. A price-shopper who asked about fees for 90 seconds and hung up counted the same as a patient booking an implant.
Nothing from the practice's patient system went back to Google. No-shows, cancellations and completed appointments worth $300–$800 all looked the same, so Google had no way to tell a good call from a bad one.
What we changed
- Compared six months of Google's call records with the practice's own appointment book.Dentrix appointment export matched against Google Ads call timestamps: 28% of converted calls had no appointment record.
- Told Google to count only appointments that actually happened.Google Ads offline conversion import: completed appointments only, uploaded daily from a Dentrix export.
- Told Google which treatments are worth the most, so it looks for more of those patients.Separate conversion action for high-value procedures (implants, Invisalign) at 3× conversion value.
What changed for the business
The calls that never turned into appointments (28% of what Google used to count) no longer teach Google who to look for.
Because Google now sees what each treatment is actually worth, the average conversion value it reports went up 340%.
The group cut its ad budget by 22% and kept the same number of patients coming in.
“Saifur found that 28% of our Google Ads 'leads' were never showing up as appointments in our system. After connecting our practice management software to Google via offline conversion imports, Smart Bidding started finding genuinely interested patients. We reduced budget by 22% and kept the same patient volume.”
